Community Health Assessment Survey

The Nassau County Department of Health is updating its community health assessment and is looking for the community's feedback to have a better understanding of the community's health needs. Please consider taking the Community Health Assessment as it will contribute to a more comprehensive and inclusive assessment. The survey will remain open until May 2, 2025.
